In vivo models for cardiorenal syndrome and mitral regurgitation

For several years, we have been carrying out an intensive research program on the pathophysiological connections between the heart and the kidney in cardiorenal syndrome (CRS) using a rat model in which an acute MI is induced by permanent ligation of the left anterior descending coronary artery on the background of chronic kidney disease (CKD) (5/6 nephrectomy carried out in two subsequent surgeries). Our preliminary data focus on four main topics:

A) We have demonstrated that CKD alone is sufficient to induce high levels of interstitial fibrosis in the LV, although cardiac function measured by echocardiography was still normal.  

B) We have recently shown that adoptive transfer of regulatory T cells at the peri-infarct region after experimental MI in rats, reduces infarct size and attenuates cardiac remodeling in the healing processes of the infarcted LV.

C) We are currently investigating the potential changes in Ca2+homeostasis, the expression levels of Ca2+-channel proteins, as well as the spatial modifications in the cellular organelles in cardiomyocytes of the insulted LV, aiming to gain understanding of the subcellular dysfunction in CRS

D) We could show for the first time that TRP Vallinoid 2 channels, which are highly abundant in neurons and inflammatory cells and mildly expressed in healthy cardiomyocytes [65], are highly expressed in the heart in both cardiomyocytes and infiltrating immune cells upon acute MI (Entin-Meer et al, unpublished data). We have also shown that HL-1 murine atrial cardiomyocytes exposed to serum free and hypoxic conditions (1% O2) responded with enhanced TRP Vallinoid 2 channel expression, as documented by flow cytometry, compared to serum free only or to full medium.

H) We have recently established a rat model for mitral regurgitation. We are going to look into the pathophysiology of changes observed the myocardium in mitral valve disease including the potential re-expression of stem cell markers upon mitral damage.

Clinical Research:

Long term follow up of patients with Coronary artery disease: A large database that includes extensive clinical information as well as a serum bank was established years ago and is called TAPAS. All patients with acute coronary syndromes or stable coronary artery disease who  undergo cardiac catheterization are included in the Registry. The patients are followed on a regular basis and the clinical as well as laboratory information is recorded. Multiple studies were published based on the rigorous database.

TAVI follow up registry:

At the Tel Aviv Medical Center we perform complex cardiac interventions including aortic valve implantations. These novel interventions are the forefront of interventional cardiology. All our patients are being steadily followed in the clinic with meticulous echocardiographic and laboratory studies. Research projects have been initiated to try and evaluate thoroughly the long term outcome of these patients.

Electrophysiology:

This area of clinical research has a worldwide recognition. The team in our department is involved in research of arrhythmia in the post MI patients, in patients with various cardiomyopathies and importantly channelopaties such and long QT, Brugada, short QT and other fascinating mechanisms.
